This privacy notice sets out how Felicity Dutton – Counselling and Coaching Services uses and protects any information that you provide when you access my services or contact the service by other means and the lawful basis under the Data Protection Act 1998 and the General Data Protection Regulations (‘GDPR’ ) (May 2018) that permits this service to do so.
Consent to hold your personal data
Felicity Dutton – Counselling and Coaching Services is committed to ensuring that your privacy is protected and will seek your explicit written consent to hold personal data about you, specifically and only for the purposes of providing a therapeutic or coaching service to you. Data you provide may be analysed for the purposes of providing you the best possible service.
If you make contact via the website or by phone you will be asked to provide certain information by which you can be identified.
You can be assured that all this information is held securely and will only be used in accordance with data protection principles and my ethical principles (contained in the British Association for Counsellors and Psychotherapists Ethical Framework for the Counselling Professions: July 2018). I will honour the confidence and trust you have placed in me to uphold this.
The basis on which I hold or share information about you is outlined below as well as information regarding the limits around confidentiality.
The types of personal data I collect and use
Whether or not you become a client of this service I’ll use your personal data for the purposes of initiating a service for you. I collect the majority of this information during first contact and initial consultation. I will advise you if any of this data is optional.
At first contact
I will collect the following information in order to establish if I can provide you with a service:
Date of birth
Contact information including your email address
Geographic information from your postcode
Other information ethically relevant to you engaging in a service with me, e.g. the name of your GP practice
Information about any disability or communication requirements you may have
I will also ask you to provide a brief description of what is bringing you to this service
This information is collected initially on a paper based record and then subsequently entered on to an electronic document which will be password protected
At initial consultation/discovery session
During therapeutic or coaching services
Anonymised notes – I will keep brief notes detailing what was covered in our sessions. Your identity is protected and these notes are not shared with anyone unless legally required to do so or you (the client) specifically requests I share them with another third party, which I have the right to refuse to do
Personal data that identifies you and your session notes are held separately and securely to protect your anonymity
These notes are the property of Felicity Dutton – Counselling and Coaching Services and not the client
What I do with the information I gather
I collect this information to understand your needs and in order to provide you with the best possible service, and in particular for the following reasons:
The lawful basis and purpose of holding this data is to ensure I can meet the terms of providing a contract for our work together. It is also to ensure good governance and that any work carried out is ethical and in your best interest.
Where I store your personal data
Felicity Dutton – Counselling and Coaching Services will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy notice and data protection and confidentiality requirements.
All information you provide is stored securely and information held on computer is password protected.
Controlling your personal information
When providing therapeutic services I may learn sensitive information about you. If any of this information means I am worried about you or have concerns regarding any risk of harm to yourself or others I may need to share this information. I would discuss this with you first wherever possible but if you do not give consent, I may still have to disclose information.
From time to time I run events, workshops and courses in collaboration with other businesses or services. If our contact with each other is via an event rather than 1:1 and I am the organiser of said event I will share your basic contact info and any info you have agreed relevant. Note that any businesses or services I work with will have their own privacy policies.
Access to information
You have the right of access to the personal information which I hold about you under the Data Protection Act 1998 and the GDPR and also the right of access to your client notes. You can contact me or email us if you wish to examine what I hold and believe that any information I am holding on you is incorrect or incomplete. Steps will be taken to correct any incorrect information that I hold.
There is no charge to access this information and it will be provided to you within one week unless circumstances require longer, but this will not exceed a month in any event.
You have the right to complain to the Information Commissioners’ Office in connection with enforcement and compliance with data protection law at www.ico.org.uk
